Report says mistakes led to death of two firefighters in Morris Heights fire

Fire officials are releasing a report Wednesday on a Morris Heights fire that killed two firefighters back in January. Six firefighters were forced to jump from the fourth floor of the apartment building, two of those men died from their injuries. A panel of five fire chiefs says communication and preparation mistakes contributed to the death of the men. While the report is critical of the mistakes, officials say it does not recommend discipline for anyone. It does say, however, that better evacuation training is needed.
